The Deputy for International Communications of the Office of the Leadership of the Islamic Republic states that Mojtaba Khamenei "despite the injury he had, is now in complete health." Mohsen Qomi described the comments regarding Mojtaba Khamenei's health as "a trick by the enemy to spread rumors" and said: "The enemy wants to provoke us into reactions with rumors about his health and to advance its conspiracies through these reactions." He claimed that his absence in any form, including audio or video messages, is "only for the sake of preserving his life."

In explaining the situation of the new leader of the Islamic Republic, Mohsen Qomi also stated that despite "the injury he had, he is in complete health, managing affairs, and handling the negotiations and field issues himself, and recently gave some warnings to the negotiating team."

The claim of "complete health" comes while The New York Times reported on April 22, citing four Iranian officials, that Mojtaba Khamenei is still undergoing treatment and is awaiting a prosthesis for one of his legs and plastic surgery to repair severe facial injuries caused by burns.

Ismail Baqaei, the spokesperson for the Islamic Republic's Foreign Ministry, criticized the reliance on Donald Trump's tweets to judge the performance of the Foreign Ministry without naming him, stating that in the current sensitive situation, "we should not turn against each other." Baqaei said it is not pleasant that the words of someone who "has a history of lying" and whose goal is "to disrupt our society" are easily used as a basis for judging the performance of the Foreign Ministry. He added that the opposing side, which "injects false information on social media," does not wish well for the Islamic Republic and acts with intent.

معاون ارتباطات بین‌الملل دفتر رهبری جمهوری اسلام، می‌گوید مجتبی خامنه‌ای «علی‌رغم مجروحیتی که داشت، اکنون در سلامتی کامل به سر می‌برد».
محسن قمی اظهارنظرها درباره وضعیت سلامتی مجتبی خامنه‌ای را «ترفند دشمن برای شایعه‌سازی» خواند و گفت: «دشمن می‌خواهد با شایعه‌سازی درباره سلامت او، ما را به واکنش‌هایی وادار کند و از این واکنش‌ها توطئه‌هایش را پیش ببرد.» و ادعا کرد عدم حضور او به هیچ شکل از جمله پیام صوتی یا تصویری «فقط به‌دلیل حفظ جان اوست».
محسن قمی در توضیح وضعیت رهبر جدید جمهوری اسلامی نیز می‌گوید وی به‌رغم «مجروحیتی که داشت، در سلامت کامل به سر می‌برد مشغول تدبیر امور است و مسائل مذاکرات و میدان را خودش تدبیر می‌کند و اخیراً تذکراتی هم به هیئت مذاکره‌کننده داد».
ادعای «سلامت کامل» در حالی است که نیوریوک تایمز سوم اردیبهشت به‌نقل از چهار مقام ایرانی نوشت مجتبی خامنه‌ای همچنان تحت درمان قرار دارد و در انتظار دریافت پروتز برای یکی از پاها و جراحی پلاستیک برای ترمیم آسیب‌های شدید صورت ناشی از سوختگی است.

اسماعیل بقایی، سخنگوی وزارت خارجه جمهوری اسلامی، بدون نام بردن از دونالد ترامپ، از استناد به توییت‌های او برای قضاوت درباره عملکرد وزارت خارجه انتقاد کرد و گفت در شرایط حساس کنونی «نباید به جان همدیگر بیفتیم».
بقایی گفت خوشایند نیست که سخن کسی که «سابقه دروغ‌گویی دارد» و هدفش «به هم ریختن جامعه ماست»، به‌راحتی مبنای قضاوت درباره عملکرد وزارت خارجه قرار گیرد.
او افزود طرف مقابل که در شبکه‌های اجتماعی «اطلاعات نادرست تزریق می‌کند»، خیر جمهوری اسلامی را نمی‌خواهد و هدفمند عمل می‌کند.
